Personal Data Processing Agreement

I hereby give my consent to the processing of my personal data by PT SURFDISCOVERY (NPWP: 21.070.116.5-905.00, address: Jl. Sriwijaya 7, Lantai 3, Kel. Legian, Kec. Kuta, Kabupaten Badung, Bali, Indonesia) and confirm that by giving such consent, I act freely and in my own interest. In accordance with Federal Law No. 152-FZ of July 27, 2006 “On Personal Data,” I agree to provide information related to my identity, including my last name, first name, patronymic, registration address, position, contact phone number, email address, passport details, and other information. Alternatively, if I am a legal representative of a legal entity, I agree to provide information related to the entity’s details: name, registered address, business activities, and the name and full name of the executive body. In the event of providing personal data of third parties, I confirm that I have obtained consent from such third parties, in whose interests I act, for the processing of their personal data, including: collection, systematization, accumulation, storage, clarification (updating or modification), use, distribution (including transfer), depersonalization, blocking, destruction, as well as performing any other actions with personal data in accordance with the current legislation of the Russian Federation.

This consent to the processing of personal data is given by me for the purpose of receiving services provided by PT SURFDISCOVERY on and through the website surfdiscovery.org.

I express my consent to the processing of all the above-mentioned personal data, including the following actions: collection, systematization, accumulation, storage, clarification (updating or modification), use, distribution (including transfer), depersonalization, blocking, destruction, as well as performing any other actions with personal data in accordance with the current legislation of the Russian Federation. Data processing may be carried out both with the use of automation tools and without their use (non-automated processing).

When processing personal data, PT SURFDISCOVERY is not limited in the methods of their processing.

I hereby acknowledge and confirm that, if necessary, PT SURFDISCOVERY has the right to provide my personal data to third parties for the purposes stated above, including when engaging third parties to provide services for these purposes. Such third parties have the right to process personal data based on this consent and to inform me about service rates, special promotions, and offers of the website surfdiscovery.org. Notification is carried out via telephone communication and/or email.
